Myeloperoxidase(MPO)isatetramericheme-containingproteinabundantlyproducedinneutrophilgranulocyteswhereitplaysanimportantanti-microbialrole.DuringdegranulationMPOisreleasedintotheextracellularspace.There,aspartoftheneutrophils“respiratoryburst”,itproduceshypochlorousacidfromhydrogenperoxideandCl–.MPOalsouseshydrogenperoxidetooxidizetyrosinetothetyrosylrADIcal.Bothhypochlorousacidandtyrosylarecytotoxicandwhenpresentcankillbacteriaandotherpathogens.Hereditarydeficiencyofmyeloperoxidasepredisposesindividualstoimmunedeficiency.
StudieshaveshownanassociationbetweenelevatedMPOlevelsandcoronaryarterydisease,andin2003itwassuggestedthatMPOmayserveasasensitivepredictorofmyocardialinfarctioninpatientscomplainingofchestpain.SincethattimetheclinicalutilityofMPOtestingincardiacpatientshasbeensolidlyestablishedintheliteraturewithwellover100paperspublished.In2010thisclinicalapplicationwasfurtherrefinedbyadditionalstudieswhichdeterminedthatmeasuringbothMPOandC-reactiveprotein(CRP)providedmoreaccuratepredictionofmortalityriskthanmeasuringjustCRPalone.
